Assistance Update. We partner with West End Medical Center, a local FQHC.
Pictured is the West End primary care mobile outreach services at one of our
sites where we are providing integrated primary care and behavioral health
services for adults with serious mental illness and other behavioral health
disorders.

Cobb & Douglas Counties Community Services Boards are partnering with a
local FQHC, West End Medical Center, to provide primary care mobile outreach
services at a SPMI-OP site. They are using funds from a Kaiser Foundation
grant award to engage in an NIMH primary care/behavioral healthcare
integration project.  The

com/>  Cobb County Community Services Board and the Douglas County Community
Services Board are public agencies created by state law to provide mental
health, developmental disability, and substance abuse services. The service
areas are Cobb, Douglas, and Cherokee Counties, Georgia. The agency provides
support to over 14,000 people annually. The agency's goals include: consumer
preference; work initiatives; and the expansion of the availability and
access to natural community supports through the reduction of stigma.
