Subject: [PC-BH-Integration] Carecoordination agreements
TheNational Council is participating in a study about care
coordinationagreements, funded by the Commonwealth Fund. The study is
being conducted bythe Center for Studying Health System Change (HSC), a
non-partisan researchorganization in Washington, DC. HSC researchers
would like tointerview primary and behavioral health organizations who
have well establishedcare coordination agreements supporting their
integration efforts. Thepurpose of this project is to learn more about
the construction, implementationand usefulness of care coordination
agreements and to share lessons learnedwith other health care providers
and policy makers. Please note that HSC willnot disclose the names of
individual respondents in any publications.
Theresearchers are looking for care coordination sites that include a
primary carepractice and where the written agreements are relatively
broad in scope, ascompared to a purchase of services agreement. Given
the attention topatient-centered medical homes and care coordination in
health care reform,it*s vital that primary and behavioral health
integration be represented inthese type of studies.
