Subject: [PC-BH-Integration] Care coordination agreements


The National Council is participating in a study about care coordination agreements, funded by the Commonwealth Fund. The study is being conducted by the Center for Studying Health System Change (HSC), a non-partisan research organization in Washington, DC. HSC researchers would like to interview primary and behavioral health organizations who have well established care coordination agreements supporting their integration efforts.  The purpose of this project is to learn more about the construction, implementation and usefulness of care coordination agreements and to share lessons learned with other health care providers and policy makers. Please note that HSC will not disclose the names of individual respondents in any publications.

The researchers are looking for care coordination sites that include a primary care practice and where the written agreements are relatively broad in scope, as compared to a purchase of services agreement. Given the attention to patient-centered medical homes and care coordination in health care reform, it's vital that primary and behavioral health integration be represented in these type of studies.
