Hi Bill:
I found the Hogg report to be very helpful...nice overview of all the
angles along with enough detail of information.  Thanks for sharing.
Do you have a strategic plan for your integrative work that you'd be
willing to share? I'm particularly interested in seeing the desired
outcomes and ultimate impact of the activities conducted within the
model and their respective indicators used for measuring success.  An
FQHC and Community Mental Health Center, for which I am currently
conducting some planning work, is orienting towards implementation of
a collaborative care model, so I'd be interested in studying the
strategic plans of others with a few years under their belt...thanks!

We've been working with the Hogg Foundation for Mental Health for
about
three years.  We're an FQHC and using the care manager model similar
to
IMPACT.  We tried co-location - it didn't work.  We've been funded for
service expansion funds from HRSA which stabilized the project.  I'll
be
glad to respond to specific requests for information -- the Hogg
website has
a lot of the model information at
