Hi there,
I would be interested to know why the co-location was unsuccessful. I developed an integrated program for a rural/frontier area covering 6000 sq miles and although not co-located in every area and the program has been successful. I'm now working at the hospital/clinic in the same area and trying to develop a co-located model within the hospital. I'd really like to know what the barriers may be going forward.

Are the "lucky" those who already have been identified and have been
in the Mental Health system? If so, how many are SMI?

Because the lucky 100 who get the service doesn't match the mission of
primary care, which serves everybody who walks through the door.  That
is one of another 300 reasons that I could cite.    Hope you are well!

We've been working with the Hogg Foundation for Mental Health for about
three years.  We're an FQHC and using the care manager model similar to
IMPACT.  We tried co-location - it didn't work.  We've been funded for
service expansion funds from HRSA which stabilized the project.  I'll be
glad to respond to specific requests for information -- the Hogg website
has
a lot of the model information at
