[PC-BH-Integration] Inviting magazine submissions

The Spring 2008 issue of National Council Magazine is focused on Primary
Care and Behavioral Health Collaboration and we’d like to include
brief stories “From the Field” on your collaboration experiences,
both successful and unsuccessful. Please submit your write-up, NOT TO
EXCEED 500 WORDS, by Wednesday, Feb 25 to our Editorial Associate,
Nathan Sprenger at NathanS at thenationalcouncil.org (call Nathan at
202.684.7457, ext. 240 with any questions).

Your write-up should include the following information
●Type of collaboration (integration, co-location, referrals and
partnerships, etc.)
●Staffing, funding, and infrastructure/IT models and challenges
●Any expertise, resources, toolkits, etc. you’ve been able to draw
from
●Program outcomes (preferably supported by data)
●What worked and what didn’t (quick lessons you’d like to share
with fellow providers)
●Next steps/future of your program

In addition to your 500-word write-up, we encourage you to provide
testimonials from consumers and family members that have been impacted
by your collaboration programs.

We’d also love to hear from you about tools and resources on
integration and on health and wellness — we’ll share the links
through our magazine “Resource Page.”
