I have formally retired from the Washtenaw Community Heatlh Organization to specifically spend more time working integration efforts across the country.  You can still reach me at reynoldk at ewashtenaw.org until June 30, 2009 and then at kreynolds at pmpcoach.com after that.  

Also, we have model of colloboration designed that defined levels of colloboration from minimal collaboration up to full integration.  Its based on work by Bill Dougherty and Mac Baird, adapted for Integrated Health.  I will work with the National Council to get it posted on the National Council website so that folks can use it to talk about think about integration. 

With fewer "jobs" now I look forward to being able to participate more actively on this list serve and direct you to National Council resources related to collaborative/integrated care.

I have another question-- Are there any models/tools written up
providing "how-to" level advice regarding:

- patient flow (for instance, if the behavioral health provider is
seeing a patient and a primary care provider wants to do a warm
handoff, how is that done? just as one example)

- staffing ratios and case loads

Yes, Bill - The primary architect of the Washtenaw model is Kathy Reynolds, who can be reached at reynoldk at ewashtenaw.org.
