Not sure if I understand what you're looking for, but I happen to have in my
files some info from the Bazelon Center. They have
(http://www.bazelon.org/issues/children/index.htm) a book/report and fact
sheets ("Way to Go: School Success for Children with Mental Health Needs")
that document "how states and school districts have successfully combined
school-wide positive behavior support (PBS) with effective mental health
services to foster a school enfironment that is conducive to learning and
improves children's lives (June 2006)"... some interesting school-based
mental health services info posted on their website (Bazelon.org)...

On another note, I'm also on the "hunt" for a model or reports.  There's
lots of info out there on "integrated care" (mental health, substance abuse,
and primary care) and there's lots of info out there about the "medical
home" (and it's related evolution from a focus on disabled children's
systems of care to a focus on client-centeredness in primary care offices).
Is there anything out there that brings those two huge pieces of work
together?  It seems that those two "tracks" of study and implementation seem
to be parallel...does anyone know of a good model or report that addresses
the design of a community medical home that is fully integrated (ie: the
concept of "medical home" being whatever the consumer defines as his/her
medical home and offers a fully integrated approach?
