Jon,
I like your PCA brief write up. You should include
www.IntegratedPrimaryCare.com in the list of websites.

Lynne,
I think a useful website for these kind of measures may be
motivationalinterviewing.org. much research has gone into showing how
MI is useful in the integrated setting and the website should have links
to find that info. As Denise stated, the PHQ-9 is a good tool as well.
We are using it in a Depression Collaborative in our health center.
Contained here is an attachment of those measures and operational
guidelines for the program; hopefully they will be of help to you. The
other aspect researchers are finding helpful, as you may already know,
is that collaborative care is more successful than therapy or meds
alone. Collaborative defined as a team approach to depression: case
management, skills development through the case manager, therapy, and
working closely with the provider.
I hope this helps-if you'd like more info, please let me know.

Hello Lynne
I don't know how long you are able to see people in the hospital, but I
wonder if you might be able to use some simple and clinically useful
outcome measures to answer the question, "are people getting better as a
result of these behavioral health services?" For example, for people who
are referred for depression perhaps you could follow the PHQ-9 and/or
the Herth Hope Index. Just a thought. Denise
