We are also partnering with an FQHC and would be interested in discussing your experience and how your model is setup.

Subject: [PC-BH-Integration] integration proposals
We are a CMHC and recently partnered with a local FQHC to pilot an integrated care location. We are working with some local and government funders to fund the project and I am curious if anyone has integration proposals they are willing to share with me or preferred delivery models that are proven effective in integrating these services.
Thanks ahead of time for any assistance.

Because of the roller-coaster economic situation of the past week, the Collaborative Family Healthcare Association as decided to extend the early bird discouted registration period for its national conference until October 10th.
The Conference is in Denver from November 6-8th. It is the leading conference in the country on the work of integrating mental health and substance abuse services into medical settings.
Look at www.CFHA.net for information and to register.

Hi everyone,
I'm a psychologist based in hospital medicine. I work at a med-surg facility, and have no outpatient work. Essentially, I'm a psychologist C/L service. I receive consults primarily from hospitalists, but also from some specialty services. At this point, patients aren't billed and
our visits are documented in the medical chart. I think I have a great job.
However, Our chief is looking for objective measures by which to review our performance. Currently, we report our annual patient contacts.
However, for our performance reviews, our activities don't "fit" the physician criteria (All CHF patients admits are followed on the CHF protocol), nor do they fit the outpatient Behavioral Health criteria (On
time for appointments).
I'm open to any suggestions anyone would like to make regarding measurable objective criteria.
Thanks
L
