I have attached the Medicare guidelines about the use of the health and
behavior codes in the Midwest region that includes my state, Wisconsin.
I would imagine there are similar criteria in other regions. It seems
that most insurance companies in Wisconsin have come to recognize and
reimburse on these codes, although they all use them slightly
differently. Here, Medicare requires that the provider billing with
these codes is a licensed psychologist, but Medicaid is much more
inclusive as to who can use these codes, with master's level providers
and psychiatrists on the list. I believe the billing is similar to
mental health codes in how it is processed, which will depend on what
kind of setting you are in.

Which states are using these codes?  Are they billing under the
behavioral practitioner's name or the identification number of the
clinic?  What sorts of behavioral practitioners are being used?  Does
the behavioral practitioner need to be independently licensed? Thanks.
