Collaboration: The Key to the Medical Home

Are you interested in collaboration? Integrated healthcare? Beautiful mountains? If so, the Collaborative Family Healthcare Association's Tenth Annual Conference may be just for you. November 6-9, 2008, in Denver, Colorado, CFHA will hold its Tenth Annual convention at the downtown Denver Grand Hyatt Hotel.  
The theme of the CFHA conference for 2008 is Collaboration as the Key to the Medical Home.  It is clear that the Medical Home concept is enjoying a surge of support, and will most likely be at the foundation of the current round of health care reform. All over the United States primary care is being retooled to produce the collaboration and continuity necessary for a medical home. CFHA's (www.CFHA.net) national conference is a perfect showcase for this agenda, and given the geographical and professional diversity of the attendees, the conference will give tremendous impetus to the development of integrated primary care both in Colorado and across the country.
Keynote speakers will include Larry Green, M.D. who will address national trends in the reform of our health care system, particularly as it pertains to the creation of integrated medical homes. On Friday morning we will have a plenary panel of Arlene Katz, from Harvard University, Sue Bergeson from the Depression and Bipolar Support Alliance, and Bill Doherty from the University of Minnesota, each bringing a unique perspective to the notion of integrating patient and community health resources into the medical home. Our Friday lunch speaker will be Ann McDaniel, Vice-President of the Washington Post Corporation, who will talk about the election that will have just occurred and its implications for health care and healthcare policy. 
You may propose one or more of the following types of presentations:
Brief Report:  			15 min.
Lecture/Discussion:   		45 min.
Panel Presentation:		90 min
Workshop:			90 min
Presentations can be in any of the following tracks:  (Note that the last track is "Other")
