[PC-BH-Integration] Utilization of the Four Quadrant Model

As the Project Manager for a BH/PC Integration Project located in 
Gloucester, Massachusetts, I am conducting research on potential 
implementation strategies of the Four Quadrant Model proposed by the 
NCCBH and Barbara Mauer.   Specifically, I am interested in learning if 
your practice utilizes screening and assessment tools to assign clients 
into Quadrants, the types of service delivery and evidence-based 
practices utilized within each quadrant, and if there have been systems 
utilized to track individual movement between quadrants based upon the 
stability of their physical/behavioral health status.  Thank you in advance.
