[PC-BH-Integration] Integration info

Good morning all,
 
I am a grant writer working with a team of three organizations (two
hospitals and a behavioral health organization) that is preparing to
apply for startup funding to integrate primary and behavioral care at
various sites, all located in rural eastern Kentucky.
 
I am seeking any help/information from organizations that have conducted
similiar integrations. How did you set up protocols for sharing patient
information? How did the referral structure/continuum of care flow? What
info belonged to which organization? 
 
Any information will be greatly appreciated. Please feel free to contact
