[PC-BH-Integration] Integration info

Thanks for the information.  We are members of the DCPCA and are in
discussions with them about integrating care models.  I was hoping for a
place that I could visit to see how some models actually run.  I will be
in Chicago on vacation which is why I mentioned that, but unfortunately
won't have time to visit Racine.

that has integrated a BHC within a primary care clinic, preferably a
FQHC?  The FQHC I work with is in DC and we currently provide mental
health services but I am interested in the BHC model and integrating

	I am a grant writer working with a team of three organizations
(two hospitals and a behavioral health organization) that is preparing
to apply for startup funding to integrate primary and behavioral care at
various sites, all located in rural eastern Kentucky.

conducted similiar integrations. How did you set up protocols for
sharing patient information? How did the referral structure/continuum of
care flow? What info belonged to which organization?
