I think it might be quicker to email Kirk Strosahl and ask if he
consulted to any of the FQHCs in the area.  I would be surprised if the
DCPCA is tracking BHCs at this point, though I think the PCAs will be

that has integrated a BHC within a primary care clinic, preferably a
FQHC?  The FQHC I work with is in DC and we currently provide mental
health services but I am interested in the BHC model and integrating

	I am a grant writer working with a team of three organizations
(two hospitals and a behavioral health organization) that is preparing
to apply for startup funding to integrate primary and behavioral care at
various sites, all located in rural eastern Kentucky.

conducted similiar integrations. How did you set up protocols for
sharing patient information? How did the referral structure/continuum of
care flow? What info belonged to which organization?
