[PC-BH-Integration] Integration info

[Meg Backas]

Does anyone know of a location in the DC metro area or Chicago that has
integrated a BHC within a primary care clinic, preferably a FQHC?  The
FQHC I work with is in DC and we currently provide mental health
services but I am interested in the BHC model and integrating that here.

South Central Foundation in Alaska has successfully integrated 9 BHCs
into family medicine, pediatrics and a rural clinic. I would be happy to
answer any questions that you may have.

Good morning all,

 

I am a grant writer working with a team of three organizations (two
hospitals and a behavioral health organization) that is preparing to
apply for startup funding to integrate primary and behavioral care at
various sites, all located in rural eastern Kentucky.

 

I am seeking any help/information from organizations that have conducted
similiar integrations. How did you set up protocols for sharing patient
information? How did the referral structure/continuum of care flow? What
info belonged to which organization? 

 

Any information will be greatly appreciated. Please feel free to contact
