Kathy Reynolds at Washtenaw Heath.Services (I think that's the name of her program) did a great job integrating CMHC and physical health care services - they're in Ann Arbor, Michigan (anybody on the listserv know their address? I think her email is kreynolds at ewashtenaw.org)
 
Also, Barb Mauer's paper for the National Council of Community BH Services on the "Four Quadrant Model" is a must read...here's a link to the NCCBH webpage that has the PDF andd other resources.

Good morning all,
 
I am a grant writer working with a team of three organizations (two hospitals and a behavioral health organization) that is preparing to apply for startup funding to integrate primary and behavioral care at various sites, all located in rural eastern Kentucky.
 
I am seeking any help/information from organizations that have conducted similiar integrations. How did you set up protocols for sharing patient information? How did the referral structure/continuum of care flow? What info belonged to which organization? 
 
Any information will be greatly appreciated. Please feel free to contact me off the list.
