Googling for IHC billing ideas, suggestions, etc, today, I came across
"Models in Asheville," a 2003 three page summary of several models of
Integrated Primary Care. Interesting. Available at

Also, "Billing for the Evaluation and Treatment of Adult Depression by
the Primary Care Clinician" (2004). Even more interesting. Available at

Also a reference to but no link to a free copy of "Funding mechanisms
for depression care management: opportunities and challenges," John
Bachman et al, General Hospital Psychiatry 28 (2006) 278-288. I'd
appreaciate a copy if anyone has one.
I also stumbled across a great paper on "Mind and Body Teunited:
Improving Care at the Behavioral and Primary Healthcare Interface"
prepared for the American College of Mental Health Administration Summit
March 2007. It's available at
