[PC-BH-Integration] care manager

Family Health Centers, Louisville, KY, would like to hear from CHCs that have been able to add a care manager with the support of a grant.  How many were in your registry when you applied?  Is anyone using a care manager to work with both depression and diabetes (or other chronic disease) patients?  If using separate care managers, how does the case load of depression care manager compare to that of care manager for other chronic disease patients. Thanks for any information you can share with us.
