      Not sure what you're asking... Are you looking for help in developing these services (e.g., organizational change, clinician training, developing operational and financial systems to support these services, etc.) or in actually providing clinical services (e.g., screening for depression, primary care based treatments, etc.)
      I'm on the Board of the Collaborative Family Healthcare Association, and you can find some ideas, resources, etc. on our website (http://www.cfha.net/).  More importantly, there's a very active group of people in Minnesota who have experience, training, etc. in pc/bh integration.  If you'd like to open up a conversation on this topic, I'd be glad to help.
