What is the data that you have collected?

Are there community health needs assessment data gathering tools, resources and/or methods that you or others have found particularly useful in the context of integrated behavioral and primary care?

Just thought I would introduce myself. My name is Michael McGlathery and I 
am the clinical supervisor in an integrated BH and primary care health 
system. This is a county health system that serves folks who are at the 
poverty level or below. Uninsured and medi-cal patients make up our patient 
population. I am starting to think about what to do with the data we have 
collected so far. Any ideas?
