I have always found  Richard Heuer's work very useful--both in theory
and in practice.  If anyone wants to know about why intelligence
sometimes fails just read his "Psychology of Intelligence Analysis"

With that in mind, what in "Part II - Cognitive Biases" of Heuer's
"Analysis" do you think applies to the Iraq WMD intelligence effort?  I
would be interested in your ideas.  I have my own of course!
